Ann T. Hummel, FNP-C

Ms. Hummel is a family nurse practitioner at CHP Lee Family Practice, where she cares for patients age 3 through adulthood. She received her undergraduate nursing degree from Russell Sage College and went on to earn a Master of Public Administration as well as a second Master of Science in Nursing. She also served as Emergency Services Director for the Albany Fire Department. She has more than 25 years of critical care, emergency and transport nursing.

Anthony Hamling, LICSW

Anthony Hamling works at CHP Lee Family Practice with the primary care and behavioral health team, providing psychotherapy for clients of all ages. He earned his master’s degree in social work from Westfield State University and completed his Bachelor of Arts in psychology at Massachusetts College of Liberal Arts. In his MSW program, he was a Massachusetts Child Welfare Institute Fellow. He has worked previously as an Intensive Care Coordinator at the Brien Center in Pittsfield and most recently as a supervisor and social worker at the MA Department of Children & Families.

Ashli Minor, MS, RDN, LDN, CLC

Ashli Minor, registered dietitian nutritionist and certified lactation counselor, is a wellness educator with a background in medical nutrition therapy. Ashli counsels adults and children, with a special focus on women’s health and all facets of integrative health care. She received a bachelor’s degree in Behavioral Neuroscience from Lehigh University, a bachelor’s degree in Human Nutrition from University of Massachusetts Amherst and a master’s in Holistic Nutrition from Clayton College of Natural Health. Ashli has worked for private and public organizations including health resorts, hospice programs, visiting nurse settings, private practice and community organizations. At CHP Ashli provides nutrition counseling on the CHP Mobile Health Unit in person and virtually. Ashli also oversees a food security grant based program which targets diabetics and pregnant women throughout Berkshire County.

Janell Hostetler, MSN, ARNP, FNP-BC

Janell Hostetler, MSN, ARNP, FNP-BC is a family nurse practitioner at CHP Lee Family Practice, caring for patients of all ages. Her clinical focus includes: adult/pediatric and LGBTQIA+ care, wellness, prevention, and mindfulness. She is certified in pediatric advanced life support and certified basic life support. She is bilingual in English and Spanish and is committed to personalized and compassionate patient care. She received a B.S. in nursing and a masters in nursing from Chamberlain University in Illinois. She is board-certified in Family Health by the American Nurses Credentialing Center (ANCC).

Jeremy Stoepker, MD

Dr. Jeremy Stoepker is a family practice physician at CHP Lee Family Practice, caring for patients of all ages. He graduated from Georgetown University School of Medicine in Washington, DC, then completed his residency training at Columbia Presbyterian in New York City. He received an award in “Patient Centered Care” and was also ranked one of New York City's top doctors for LGBTQ health.  His interests/strengths are in LGBTQ health, Alternative/Complementary medicine, and holistic care of individuals and whole families. Lauren Young, FNP

Lauren Young, family nurse practitioner, sees patients of all ages at CHP Great Barrington Health Center. A graduate of the CHP Family Nurse Practitioner Residency Program, Ms. Young earned her M.S. in nursing/family nurse practitioner from Simmons University, and she holds a master’s degree in health promotion from Mississippi State University. She earned her B.S. in dietetics from Colorado State University. She completed family nurse practitioner rotations at medical practices in the Plymouth, N.H. area. and at the New Hampshire Veterans Home before joining CHP.  In her free time, she enjoys traveling, skiing, boating in NH with friends and family, and working on her new home.

Melanie Levitan, MD

Dr. Levitan is a family medicine physician at CHP Lee Family Practice. She has been caring for children and adults in Berkshire County for more than three decades. After receiving her undergraduate degree from the University at Albany, she earned her M.D. from Vanderbilt University School of Medicine. She completed her family practice residency at St. Clare’s Hospital in Schenectady, N.Y.  Dr. Levitan has studied complementary medicine including nutrition, stress management techniques and homeopathy, and she offers these modalities to her patients.

Michael Kaplan, MD, FAAFP

Dr. Kaplan, family medicine physician, has more than three decades of experience caring for patients from birth through elder years. He received his undergraduate degree from Columbia College and his medical degree from the University of Rochester School of Medicine and Dentistry. He completed his residency in family medicine at Middlesex Memorial Hospital, an affiliate of the University of Connecticut. In 1980, he became board certified in family medicine and in 1988, he earned additional certification in geriatric medicine.

Sharon Bigler, DNP

Sharon K. Bigler, a family nurse practitioner, sees patients of all ages at CHP Lee Family Practice. She earned her doctorate of nursing practice at the University of Tulsa (OK), where she also received her bachelor of science in nursing. Prior to earning her DNP, Sharon was a critical care nurse, working for 15 years in medical-surgical, cardiovascular, burn, and neurotrauma intensive care units. Earlier, she worked for two years as a registered nurse case manager. She and her husband reside in Pittsfield.

Terrance “TC” Clarke, FNP

Terrance "TC" Clarke is a family nurse practitioner at CHP Lee Family Practice, where he cares for adults of all ages and children over age 2.A graduate of the University of Massachusetts School of Nursing, he has worked previously in an urgent care setting and caring for patients with substance use disorders. He has also worked as a charge nurse in the Massachusetts state prison system, and he also served as a Peace Corps volunteer. A resident of Lenox, he enjoys hiking and helping behind the scenes at local theaters.
