Ann T. Hummel, FNP-C
Ms. Hummel has more than 25 years of critical care, emergency and transport nursing. She received her undergraduate nursing degree from Russell Sage College and went on to earn a Master of Public Administration as well as a second Master of Science in Nursing. She also served as Emergency Services Director for the Albany Fire Department.

Annie Schwartz, MS, RDN, LDN
Annie is a clinical nutritionist seeing CHP patients at Great Barrington Health Center and via telehealth. A graduate of Columbia University Teachers College, she holds a master’s degree in nutrition and exercise physiology. Annie is a Health at Every Size (HAES)® aligned clinician and is currently pursuing additional training to become a Certified Body Trust® Provider through The Center for Body Trust® as well as to become a Certified Intuitive Eating Counselor from the Original Intuitive Eating Pros®.

Ashli Minor, MS, RDN, LDN, CLC
Ashli Minor, registered dietitian nutritionist and certified lactation counselor, is a wellness educator with a background in medical nutrition therapy. Ashli counsels adults and children, with a special focus on women’s health and all facets of integrative health care. She received a bachelor’s degree in Behavioral Neuroscience from Lehigh University, a bachelor’s degree in Human Nutrition from University of Massachusetts Amherst and a master’s in Holistic Nutrition from Clayton College of Natural Health. Ashli has worked for private and public organizations including health resorts, hospice programs, visiting nurse settings, private practice and community organizations. At CHP Ashli provides nutrition counseling on the CHP Mobile Health Unit in person and virtually. Ashli also oversees a food security grant based program which targets diabetics and pregnant women throughout Berkshire County.

Diane Singer, MD
Dr. Singer earned her undergraduate degree from Colgate University and then attended the New Jersey Medical School of Rutgers University. She completed her internship in internal medicine, her residency in ophthalmology and a fellowship in cornea and external disease—all at the University of Rochester Department of Medicine. Dr. Singer’s extensive experience in eye care includes serving as a clinical assistant professor of ophthalmology at University of Rochester Eye Institute. At the Atlanta, Georgia Veterans Administration Medical Center, she staffed the VA’s walk-in vision clinic.

Janell Hostetler, MSN, ARNP, FNP-BC
Janell Hostetler, MSN, ARNP, FNP-BC is a family nurse practitioner who will see patients of all ages. She received a Bachelor of Science in Nursing (BSN) and a Masters in Nursing (MSN) from Chamberlain University in Illinois. She is board-certified in Family Health by the American Nurses Credentialing Center (ANCC). As a family trained nurse practitioner, she provides care for patients of all ages. Her clinical focus includes: adult/pediatric and LGBTQIA+ care, wellness, prevention, and mindfulness. In addition to her registered nursing and family nurse practitioner certification, she is certified in pediatric advanced life support, and certified basic life support. She is bilingual in English and Spanish and is committed to personalized and compassionate patient care.

Jeremy Stoepker, MD
Dr. Jeremy Stoepker graduated from Georgetown University School of Medicine in Washington, DC, then did his residency training at Columbia Presbyterian in New York City. He received an award in “Patient Centered Care” and was also ranked one of New York City's top doctors for LGBTQ health. His interests/strengths are in LGBTQ health, Alternative/Complementary medicine, and holistic care of individuals and whole families.

Melanie Levitan, MD
Dr. Levitan has been providing health care to children and adults in Berkshire County for more than three decades. After receiving her undergraduate degree from the University at Albany, she earned her M.D. from Vanderbilt University School of Medicine. She completed her family practice residency at St. Clare’s Hospital in Schenectady, N.Y. Dr. Levitan has studied complementary medicine including nutrition, stress management techniques and homeopathy, and she offers these modalities to her patients.

Michael Kaplan, MD, FAAFP
Dr. Kaplan, family medicine physician, has more than three decades of experience caring for patients from birth through elder years. He received his undergraduate degree from Columbia College and his medical degree from the University of Rochester School of Medicine and Dentistry. He completed his residency in family medicine at Middlesex Memorial Hospital, an affiliate of the University of Connecticut. In 1980, he became board certified in family medicine and in 1988, he earned additional certification in geriatric medicine.

Rachel Duvall, LICSW
Ms. Duvall is a licensed independent clinical social worker working with clients of all ages. She earned her master’s degree in social work from New York University’s Silver School of Social Work, and she holds a master of fine arts in acting from the National Theatre Conservancy in Denver, Colo. She earned her bachelor of science in theater performance, with associated study in psychology, from the University of Evansville in Indiana, where she graduated with honors. She has worked in both private and community health settings prior to joining CHP. She is based at CHP Lee Family Practice.

Rita Willey, DNP
Rita Willey, DNP has been caring for Berkshire County residents for over 25 years. She earned her doctorate of nursing (DNP) and her master’s degree in family nurse practitioner (FNP) from the University of Massachusetts at Amherst. Willey holds her master’s in business administration, with a concentration in health care management, from Western New England College. She earned her bachelor’s degree in nursing from UMass and completed her associate degree in nursing at Berkshire Community College. A resident of Lenox, she has volunteered with the Berkshire County Military Family Support Group, a program of the American Red Cross during the Iraq War from 2003-2010. She is an active volunteer member for 26 years with the National Ski Patrol and holds certification in outdoor emergency care.

Terrance Clarke, FNP
Terrance "TC" Clarke is a family nurse practitioner at CHP Lee Family Practice, where he cares for adult patients. A graduate of the University of Massachusetts School of Nursing, he has worked previously in an urgent care setting and caring for patients with substance use disorders. He has also worked as a charge nurse in the Massachusetts state prison system, and he also served as a Peace Corps volunteer. A resident of Lenox, he enjoys hiking and helping behind the scenes at local theaters.
