Dear Friend,
CHP’s programs and services address underlying factors that impact the health of children and adults throughout their lifetime and are structured to give your family, friends, and neighbors the best possible opportunity to live healthy lives.
Every year, CHP helps hundreds of families that fall on hard times and need help getting back on track.
When people are worried about where to sleep tonight, if they’ll be able to feed their children, or how to pay next month’s rent, they can’t focus on basic health needs. That’s why helping patients address the underlying issues impacting their health and well-being is imperative, for example:
♦ A single father of two suffered a devastating house fire is helped with rent, reconnecting utilities, and getting the whole family back on track with medical and dental care.
♦ An elderly patient, blind and homeless, is provided stable housing by covering their first, last, and security deposit and establishing the help of a Personal Care Assistant to navigate his day-to-day needs.
♦ A young couple expecting their first baby and facing homelessness is helped to arrange temporary housing until a permanent solution is found, provided food supplies and prenatal care, and continues to receive care for the whole family after the birth of a healthy baby.
♦ A patient struggling with alcoholism asked for help and is connected to the right resources and taking the first steps toward a brighter, healthier future.
