LEGAL NOTICE REQUEST FOR SERVICES

Community Health Programs Inc. (CHP) is soliciting a Request for Services for a “Designer” to provide Designer Services for the fit-out of a dental suite at their office located at 444 Stockbridge Road, Great Barrington, MA 01230.  The space to be fit out is approximately 3000 SF and will include: 7 operatories (120-180 Square feet each), a Manager’s Office (100 Square feet), Pano Machine space (x-ray machine), Clean Room, Dirty Room, Lab, Shared Space for Dentists and Dental Assistants, Waiting Room, and a Reception Area.

Deadline for Questions:
All questions regarding this RFS should be addressed exclusively in writing to Ms. Rachel Rose, Project Manager CHP, no later than 2:00 PM on February 16, 2023.

Deadline for Proposals:
Proposals will be received at the office of Ms. Rachel Rose, CHP, 444 Stockbridge Road, Great Barrington, MA 01230. Proposals that will be considered for qualification should be submitted no later than Thursday, February 23, 2023, at 2:00 PM.

Proposals must be clearly identified by marking the package or envelope with the following: ‘Dental Office Fit out Design’ followed by ‘Name of Applicant.’

Specifications and Request for Services documents may be obtained by e-mail (rrose@chpberkshires.org) from Ms. Rachel Rose, Project Manager, on or after February 1, 2023.

CHP reserves the right to accept or reject any or all Proposals received, as deemed by them to be in the best interest of the Owner, and its decision shall be final.

SPECIAL NOTE: Interested parties are invited to attend a pre-proposal building tour and conference to be held at the CHP’s Facility at 444 Stockbridge Road, Great Barrington, MA 01230, at 2 PM on Thursday, February 9, 2023.
