CHP Mobile Health and practice locations now offer COVID-19 (Pfizer) vaccines for patients of all ages (6 months +). If you have
insurance, please bring it with you.
Appointments are suggested; walk-ups are welcome.

Whenever our Mobile Health Team is on the road, we can care for you with sick visits, minor injury care, and so much more.

CHP’s Mobile Health Team provides primary care and urgent care services, including:
- Minor Sickness, Infections, Injuries, UTIs, STIs
- COVID-19 & Flu Vaccines/Testing
- Annual Physicals, Gynecology & Birth Control
- Preventative Care, Primary Care Referrals, Health Education

FLU
- The flu vaccine is available for all ages six months and older.
- The senior flu vaccine is available for adults over 65.
COVID-19
- Updated COVID-19 vaccines (Pfizer) are available for all patients six months and older.
- Parents and caregivers should call CHP for pediatric vaccine appointments. We want to be sure your child is ready for vaccines based on prior vaccinations.
- If you have health insurance, please bring your current insurance information and vaccine card to your vaccine visit. However, vaccinations with CHP Mobile Health are available to all, regardless of ability to pay.
