Are you a Community Health Programs patient who uses MassHealth for your health care?
If so, on March 1, 2018, Berkshire Fallon Health Collaborative, or BFHC, will be your new health plan.
The good news is that all CHP doctors and nurse practitioners are part of BFHC, so you’ll keep seeing them in their offices, just as you do today.
If you are insured by an employer health plan, private insurance, subsidized insurance (“Obamacare”), Medicare or other non-MassHealth benefits, your coverage remains unchanged. Only MassHealth patients are impacted.
Berkshire Fallon Health Collaborative (BFHC) is a MassHealth Accountable Care Organization (ACO) Partnership Plan. It’s made up of CHP, Berkshire Health Systems, several independent Berkshire County physicians and Fallon Health, a non-profit health care services company headquartered in Worcester.
Why is this happening?
An ACO is a health plan that is accountable to give quality care and help you meet your health goals. In an ACO your PCP and their team is responsible for working with you and your other Clinicians. They’ll help coordinate your care and connect you with available services and supports. This coordination will help you get the right care at the right time.

Do I have to do anything?
No. If you currently have a PCP that is participating in BFHC, then MassHealth has enrolled you in BFHC as of March 1, 2018. You shouldn’t need to do anything else.
If you’re not sure if you’re enrolled, contact your local CHP primary care practice.
