Annie Schwartz, MS, RDN, LDN

Annie is a clinical nutritionist seeing CHP patients at Great Barrington Health Center and via telehealth. A graduate of Columbia University Teachers College, she holds a master’s degree in nutrition and exercise physiology. Annie is a Health at Every Size (HAES)® aligned clinician and is currently pursuing additional training to become a Certified Body Trust® Provider through The Center for Body Trust® as well as to become a Certified Intuitive Eating Counselor from the Original Intuitive Eating Pros®.

Barbara Snyder, MD

Pediatrician Dr. Barbara Snyder specializes in adolescent medicine. She was chief of adolescent medicine at the Robert Wood Johnson Medical School in New Jersey from 1990-2015. Snyder, who also holds a master’s degree in social work from Columbia University, is a Fellow with the American Academy of Pediatrics and is chair of its executive committee on Adolescent Health, among other professional affiliations. She has worked at several academic medical centers, as well as in the U.S. Air Force in Europe. Her community work includes past board memberships for organizations focused on eating disorders, sexuality education, LGBT family advocacy, suicide prevention and teen pregnancy.

Claire A. Horth, MD

Dr. Horth, family practice physician, graduated cum laude from the University of Notre Dame. She earned her medical degree from the University of Massachusetts School of Medicine in Worcester and trained at Memorial Hospital of Rhode Island. Her wide ranging medical interests include preventative medicine, adolescent and women’s health.

Cornelia Lenherr, MD

Dr. Lenherr is a family practice physician at CHP Great Barrington Health Center. She is certified in both functional medicine and family medicine, and her practice incorporates muscle release therapy, psychoanalysis, stress reduction, mind-body medicine, meditation, biofeedback, and other health management techniques. She has previously worked at Canyon Ranch in Lenox and private practice in Rochester, NY, and at student health centers at the University of Santa Cruz and the University of California, Berkeley. Dr. Lenherr earned her medical degree at Dartmouth Medical School and she received her bachelor’s in pre-medical studies at Simon’s Rock College of Bard. She completed her family medicine residency at the University of Rochester/Highland Hospital. She also completed a fellowship in family systems medicine at Highland Hospital, focusing on family therapy training.

Diane Singer, MD

Dr. Singer earned her undergraduate degree from Colgate University and then attended the New Jersey Medical School of Rutgers University. She completed her internship in internal medicine, her residency in ophthalmology and a fellowship in cornea and external disease—all at the University of Rochester Department of Medicine. Dr. Singer’s extensive experience in eye care includes serving as a clinical assistant professor of ophthalmology at University of Rochester Eye Institute. At the Atlanta, Georgia Veterans Administration Medical Center, she staffed the VA’s walk-in vision clinic.

Erin Pacheco, MS, RDN, LDN, CLC

Nutritionist Erin Pacheco, registered dietician and certified lactation counselor, has special expertise in childhood and adolescent weight management. She earned a B.S. in health studies from Boston University and earned her M.S. in nutrition at the University of Vermont. Ms. Pacheco works with CHP patients in need of wellness support for primary health care as well as more complex medical conditions, and she also works with WIC clients.

Jennifer Rubino, DNP, FNP-BC

Dr. Jennifer Rubino, DNP, FNP-BC, provides primary care for adults and children and is based at CHP Great Barrington Health Center. Dr. Rubino is a 2022-23 resident in the CHP Family Nurse Practitioner Residency Program, specializing in women's health. She completed her Doctor of Nursing practice at Our Lady of the Elms College, where she also earned her bachelor's degree in nursing. Earlier she earned her associate degree in nursing from Maria College in Albany.

Most recently, Rubino worked as a community and rural health nurse with the CHP mobile health unit, and previously she worked at the Center for Motivation and Change in New Marlborough, providing nursing care to patients in the addiction treatment setting. She began her nursing career as CNA through the American Red Cross and has also worked in long-term care settings. She has also worked for the Dermatology Center in the Berkshires and at Sharon Hospital in Sharon, Conn. In the community, she has volunteered with Volunteers in Medicine (VIM), and she is board certified by the American Association of Nurse Practitioners.

Lara Setti, MD, MPH, FAAFP

Dr. Setti earned her B.A. in human development and social relations at Earlham College in Richmond, IN. She completed her M.D. at the School of Medicine, University of North Carolina Chapel Hill and then attended Harvard University, where she earned her M.S. in public, with a focus on international public health. Dr. Setti completed her family medicine internship and fellowship at Ventura County Medical Center in California, and in 2003, served as chief resident there.

Laura Gariepy, FNP

Laura Gariepy, family nurse practitioner, provides primary care to people of all ages in the Great Barrington area. After earning her undergraduate degree from George Washington University, she earned her M.S. in nursing at Simmons College School of Nursing and Health Sciences in Boston. She worked as a nurse at Lee Family Practice prior to joining the Community Health Center in Great Barrington. Ms. Gariepy’s clinical interests include preventative care and wellness, as well as adolescent and women’s health.

Lauren Young, FNP

Lauren Young, family nurse practitioner, sees patients of all ages at CHP Great Barrington Health Center. She earned her M.S. in nursing/family nurse practitioner from Simmons University, and she holds a master’s degree in health promotion from Mississippi State University. She earned her B.S. in dietetics from Colorado State University. She completed family nurse practitioner rotations at medical practices in the Plymouth, N.H. area. and at the New Hampshire Veterans Home before joining CHP.  In her free time, she enjoys traveling, skiing, boating in NH with friends and family, and working on her new home.

Lisa Nelson, MD

Dr. Lisa Nelson is a University of Massachusetts School of Medicine graduate and completed her residency and internship in family medicine at Boston Medical Center and South Boston Community Health Center. She has worked in private practice family medicine since 2005, and also serves on the faculty of the UMass Medical School and the University of New England College of Osteopathic Physicians. She is director of medical education at Kripalu Center for Yoga and Health and also serves as medical director of The Nutrition Center. Dr. Nelson integrates lifestyle approaches for prevention and treatment of chronic diseases through changes in diet, exercise, and stress management. Dr. Nelson leads two group medical programs at CHP,  "Healthy Eating, Healthy Living" and "Managing Stress During Stressful Times."  For more information on group medical visits, please contact your clinician.  Dr. Nelson is also co-author of Yoga & Diabetes.

Michelle Sisk, BS, RDN, LDN, CDN

Michelle Sisk is a culinary nutritionist with the CHP Nutrition Club, a nutrition support program serving clients of MassHealth, and she also works with the CHP Mobile Health team. A graduate of Marymount College of Fordham University, Sisk holds a bachelor of science degree in nutrition, and she completed her dietetic internship at the University of Connecticut and Hartford Hospital. She also earned a bachelor of arts degree in English from William College.  She also holds nutrition counseling positions at Hotchkiss School in Connecticut and at Millbrook School in Millbrook, N.Y., and she has also designed and taught a nutrition curriculum for students at Salisbury Central School, where she is also part of the school's Wellness Committee. She has also provided private in-home nutritional counseling to patients, and also worked as an outpatient dietitian affiliated with Manchester Memorial Hospital in Connecticut.  She also holds a certificate in adult weight management and is a member of the Academy of Nutrition and Dietetics and the Connecticut Dietetic Association. She has also worked as a yoga instructor, cross-country coach, and English teacher. Yoga, hiking with her family, baking, and cooking are among her personal interests.

Stephanie Taylor, PNP

Stephanie Taylor, pediatric nurse practitioner, provides primary pediatric care to children, teens and young adults up to age 22. After receiving her B.A. from Smith College, she earned her M.S. in nursing from Yale University School of Nursing. She received clinical training at the Hospital of St. Raphael in New Haven, Conn., in the Pediatric Primary Care Center and post-partum units. Ms. Taylor’s extensive experience includes rural health and community outreach.

Stephen Alsdorf, MD

Dr. Stephen Alsdorf, family practice physician, is a graduate of UMass Medical School. He completed his internship and residency in the Boston area, at Boston University Family Medicine Program, Boston Medical Center and Codman Square Health Center. Dr. Alsdorf is a member of the Massachusetts Academy of Family Physicians and a member of the Medical Executive Committee of Berkshire Medical Center, where he is also on staff. He is a diplomate of the American Board of Family Medicine and a member of the American Academy of Family Physicians.
