Sliding Fee Scale

CHP Berkshires is committed to caring for ALL patients, regardless of income or ability to pay, so we offer a Sliding Fee Scale to individuals and families who qualify.

Depending on your income and family size, a Sliding Fee Scale reduces the cost of your healthcare. The application process is easy and can start with a simple phone call to our Financial Assistance/Enrollment team, 413.854.2816.

Sliding Fee Criteria

Your “family” includes a mother, father, children, domestic partner, husband, wife, and any dependent minor who is supported by the family unit and benefits from the combined household income. Roommates do not qualify as family.

How Does a Sliding Fee Scale Work?

Anyone can apply for our Sliding Fee Scale. Your application will be processed using the U.S. Department of Health & Human Services income guidelines.

What Else Should You Know About Sliding Fee Scale?

  • The Sliding Fee Scale applies only to all services provided directly by CHP.
  • The Sliding Fee Scale must be renewed on a quarterly basis and is retroactive for up to six (6) months.
  • All application information collected is private and confidential and is only stored in the secure CHP Berkshires database. This information is not shared with any third party.
  • The Sliding Fee Scale does not apply to certain equipment or hardware, such as dentures and dental hardware.
