CHP On Wheels:
Bringing Health Care to You
Community Health Programs’ CHP on Wheels now offers primary care: routine physicals, wellness visits, chronic disease management, and sick visits on its mobile health unit—a recreational vehicle that has been designed for health care. CHP on Wheels travels to Lee and New Marlborough every month and to Otis every other month with a Family Nurse Practitioner on board who can provide care. CHP on Wheels also offers free health screenings, health education, and access to MassHealth, SNAP (formerly food stamps), and WIC.
CHP on Wheels is one of the few mobile medical units in the state that can offer physicals, disease management, acute care, and health assessments off site.
By going to small towns and villages, places that are far from health centers and from public transportation, CHP on Wheels removes barriers to care and improves the health of Berkshire County’s most vulnerable residents. It plays an essential role in CHP’s success in connecting hard-to-reach populations with health care. CHP on Wheels opens the door for innovative outreach that combines direct health care with enrollment assistance in rural settings from town halls, libraries, Councils on Aging, subsidized housing and food pantries. CHP on Wheels provide disease management, routine physicals, and acute care to those who are isolated by geography, health, or lack of money.
CHP on Wheels offers Vision Care. Thanks to the Massachusetts Medical Society and Alliance Charitable Foundation, Community Health Programs has set up an exam room for ophthalmology (eye care) on our mobile outreach van. CHP patients can receive routine eye care from Dr. Diane Singer. We offer screening, diagnosis, and treatment for most eye diseases at our three primary health facilities.
