Overview

The CHP Neighborhood Health Center, a Federally Qualified Health Center, is a comprehensive medical practice providing primary medical care in Berkshire County that is accessible to all people, regardless of insurance status, immigrant status, or ability to pay. We provide preventive, acute, and ongoing medical care for infants, children, adults and elders, and we provide full, 24-hour medical coverage for our patients, 365 days a year.

Members of our Provider Team of physicians and nurse practitioners are certified in Internal Medicine, Pediatrics and Family Practice. Each patient chooses one of these providers to be their Primary Care Provider, and the entire staff is available to meet the many health care needs of patients:

  • Evaluation and Treatment
  • Ongoing care for medical problems
  • Comprehensive care for chronic conditions
  • Referrals for specialty care and mental health care
  • Patient and parent education
  • Same day sick visits
  • After-hours coverage by a local physician
  • Adolescent care – 15 years of age & up
  • Gynecological care/family planning
  • Immunizations
  • Nutrition and wellness counseling

After Hours Care: We have 24 hour Emergent coverage through an answering service. If your call is regarding an appointment, referral, billing or prescription refill, we ask you to call during normal operating hours.

Prescription Refills: 48 hours notice for refills is required. No prescriptions are refilled after hours or on weekends. NO NARCOTICS ARE PRESCRIBED WITHOUT AN APPOINTMENT.

Forms: We are happy to fill out physical forms, camp forms, college forms if you have had your yearly physical. Please give the office at least one week to complete and return the forms to you. Otherwise, you may bring them with you at your physical appointment.

CHP Neighborhood Health Center Residency Program
The CHP Neighborhood Health Center benefits from a long-term relationship with Berkshire Medical Center’s Internal Medicine Resident Practice, accredited through the Accreditation Council for Graduate Medical Education (ACGME). Under the direction of David Albert, residents in the 3-year program provide continuing care at the CHP Neighborhood Health Center.

The CHP Neighborhood Health Center is partially funded by the U.S. Department of Health and Human Services Health Resources and Services Administration, the MA Department of Public Health, the MA Executive Office of Health and Human Services, the MA League of Community Health Centers, Elder Services of Berkshire County, and other grants and donations.
